Hola Janet: Las especies de Acinetobacter están ampliamente distribuidas en tierra, agua y ocasionalmente se pueden encontrar en piel, membranas mucosas, secreciones, y en el ambiente intrahospitalario. A menudo son bacterias comensales pero ocasionalmente causan infecciones nosocomiales. Acinetobacter baumannii se ha aislado de sangre, esputo, piel, líquido pleural y orina. Cuando se asocia a neumonía nosocomial es debido a menudo porque se originan en el agua de los vaporizadores. En personas con bacteremia por este microorganismo, los catèteres intravenosos son la fuente principial de infecciòn. En pacientes con quemaduras graves o con deficiencias inmunes, actùa como un patógeno oportunista y puede provocar sepsis. En infecciones vaginales es importante descartar la presencia de Acinetobacter porque puede presentar morfología semejante a la de Neisseria gonorrhoeae. Causa diferentes infecciones como neumonía (relacionada con mayor frecuencia con tubos endotraqueales o traqueostomía) en pacientes internados en unidades de terapia intensiva, también causa endocarditis, meningitis, infecciones de piel y heridas, peritonitis (pacientes con diálisis peritoneal), así como infecciones del tracto urinario. También se han encontrado como responsables de casos esporádicos de conjuntivitis, osteomielitis y sinovitis. Tienen un papel muy importante en la colonización e infección de pacientes hospitalizados, infecciones nosocomiales como bacteremia, infecciones del tracto urinario y meningitis secundaria; eso la hace una de las bacterias màs frecuentemente responsables de las infecciones adquiridas en el hospital. En personas inmunodeficientes puede causar también infecciones de tipo nosocomial (del tracto urinario, neumonía y sepsis). Factores de riesgo como el alcoholismo, tabaquismo, enfermedad pulmonar crónica y procedimientos invasivos en el hospital; facilitan la infección por esta bacteria. Acinetobacter baumannii puede ser adquirida por contacto persona - a - persona, a través del contacto con superficies contaminadas en unidades de cuidado intensivo, o como resultado de heridas contaminadas con suciedad y tierra. En cuanto a la terapia antimicrobiana es una tema muy complicado....pues tienen una elevada resistencia natural a los antibióticos y son capaces de adquirir resistencia contra otros antibióticos con facilidad. Existe resistencia prácticamente universal a la Penicilina, Ampicilina y Cefalotina; y la mayoría de las cepas son resistentes al Cloranfenicol, además ha aumentado la resistencia a los aminoglucósidos en los últimos años. Se ha encontrado sensibilidad variable a las Cefalosporinas de 2ª y 3ª generación y la sensibilidad a cotrimoxazol también es variable. El tratamiento combinado con un aminoglucósido mas Ticarcilina o Piperacilina es sinérgico y puede ser eficaz en las infecciones graves. También responden a la Gentamicina, Amikacina o Tobramicina en algunos casos, inicialmente el Imipenem fue efectivo pero en los últimos años han aumentado los casos de cepas resistentes a este antimicrobiano.
