Sherlock (Jonny Lee Miller) es un ex-consultor de Scotland Yard, que llega a Nueva York después de pasar un período en un centro de rehabilitación.

Obligado por su padre rico para compartir su apartamento con la Sra. Joan Watson, una cirujana que perdió su licencia hace tres años cuando uno de sus pacientes murió, él necesita mantenerse sobrio y fuera de las drogas.

Por lo tanto, Watson se une a Sherlock en su trabajo como consultor de la policía de Nueva York. El contacto de Sherlock con el departamento es el Capitán Tobias Gregson (Aidan Quinn), a quien conoció en Londres...
