Will Gerard (Nicolas Cage) es un profesor felizmente casado con Laura (January Jones). Llevan una vida tranquila, hasta una horrible noche en la que Laura es víctima de una brutal agresión sexual. Mientras Will espera nervioso en el hospital a que lo pongan al tanto del estado de Laura, un misterioso tipo llamado Simon (Guy Pearce) se dirige a él y le ofrece hacer justicia sobre la marcha y ahorrar a la pareja el suplicio de tener que pasar por un proceso judicial emocionalmente doloroso. Al verse obligado a tener que tomar una decisión en el momento, en su alterado estado de ánimo, acepta la propuesta, y se ve metido en una organización justiciera clandestina, lo que lleva a consecuencias cada vez más aterradoras y peligrosas.
